Instead from this primary physician another specialist is referred from whom you might seek the necessary treatment. The specialist may or may not be present in the list offered by the insurance policy, if the name of the doctor is not provided by the list the patients might take the advice from some other specialist as well.

PPO is considered an alternative to managed care plan and medical indemnity insurance because when the policy holders take the advice of the doctors from outside the list which is offered the insurer is liable to pay some parts of the expenditure

PPO thus appears to be the best option for those who want freedom of choice which is offered by the medical indemnity insurance and at the same time wants to get the best of advice. In these policies the constraints of the policy holders are also taken care of.
